That is Phoenix's LSL-Client Bridge. Once its failed to attach, the Bridge gets dropped as a phantom invisi-prim that is almost impossible to grab and delete. One of these will be rezzed with every login, so they can pile up, which is about the only adverse effect in that they count against your sim's prim allowance. You have to use the click-drag in Edit to 'surround' them to be able to grab them.
Edit your Preferences, Phoenix tab and disable the LSL-Client Bridge. Phoenix works fine without it.
