Thanks, I was able to isolate the problem. It seems mediaitems.create has never ever worked for non-gadgets with 3LO. I'm sorry to disappoint you.
I can't promise you this will be done too soon, the list of urgent things to fix is still very long, but I'm increasing priority on this bug.
