The last item you list is unlikely to have caused this issue.
OK, it was just speculations from my side. When one image-update go
for a long time and crash, and the next image-update fails pretty
fast, I suspect the previous image-update to be guilty.
Normal pgk install works BTW.
image-update relies on beadm where most of your woes appear to be
centered.
Please file a bug at defect.opensolaris.org.
In the meantime, I'm unable to offer any useful suggestions.
