Did you install the live upgrade packages from snv_99 *before*
upgrading?

This is required, and has always been required (though apparently not
publicized well enough given how many people have run in to this
particular problem).  You always upgrade the live upgrade packages from
the version you are upgrading *to* prior to doing the actual upgrade.
