Probably by getting OpenSSL 0.9.4. I've added a fairly complete
programmers guide to the new X509V3 code now in openssl.txt. 

X509V3_add_standard_extensions() is right at the top and a description
of X509V3_EXT_cleanup() which you do indeed use to cleanup the stuff.

I applogise for breaking the spirit of things by actually documenting
something :-)
