Point taken it was just a test, and not a very good one :) What I really
want to achieve is the user name (CN) followed by an underscore and an
Oracle generated sequence number. i.e. flintstone_7787029872983475.

Ahh I get it I just add uid to the policy section (policy_anything in my
case) to give:

[ policy_anything ]
countryName             = optional
stateOrProvinceName     = optional
localityName            = optional
organizationName        = optional
organizationalUnitName  = optional
commonName              = supplied
uid                     = optional
emailAddress            = optional

And it works - my certs now include the uid in the Subject name.
