"prefix_delegation" is a special subnet_pool ID and we cannot get the subnet pool. I believe this is the cause of your error. I think the horizon needs to mirror the special meaning.
At the moment, I have no environment with prefix-delegation enabled IPv6 environment. Could you share the output of the following command? Using some pastebin is preferred (like paste.openstack.org) I can propose a fix on this.
