I was suffering with the second and third problems, empty kcontrol & KDE apps missing from the menus, and I seem to have resolved them by changing my desktop manager to kdm, from gdm.
Of course, it could have just fixed itself with a factory update/reboot but the timing was fairly compelling. Worth a try.
