What I have done is have postfix send mail through (one of) my providers
smtp server. Recieving is done by simple fetchnmail pop3.

The only thing I needed to change was (I think) "set envelope_from=yes" in
.muttrc, because otherwise the reply would go to the wrong place somehow.
