I think you have it nailed now.  Thanks for reporting back the status.
I recently had a front panel USB connector fall apart.  That is a black
plastic
piece came out with the USB plug I inserted.  It's defective, nothing I
can do
but replace if I can find a replacment.  Otherwise, throw it away and
I'll use
a hub off the back of the computer where I can sit the hub on top.
