Well, I don't know the details, but the name "vanilla" implies that this is the vanilla kernel from kernel.org while the HEAD directory contains the SuSE kernel and kernel sources (i.e. vanilla kernel with additional SuSE patches). Therefore, the answer to your question is: you need to decide which kernel you would like to use and then choose the corresponding directory. In any case, do not replace your existing kernel, install any new kernel in addition to the old one.
