So you tried with two different DVDs/copies of 10.3 ?
The cable could be defective or the SCSI jumpers wrong.

Maybe the DVD is now defective ?
Scratches, dirt, damage to one of its inner layers (water; corrosion,
heat, bending, detaching, sun) are possibilities.
