This may have been due to the server crashing overnight. We restored from backups, but any issues created in the 12 hours or so before that may have been lost.

However, there I have seen the occasional duplicate key, and we are trying to track down how this could occur.

If the former issue was deleted, then Jira should not reuse the same key for something different or all watchers should be notified about the removal.
If not deleted, the bug is even more worse and should be investigated!
