I was never successful using the generic NV driver, I have to use the proprietary nvidia driver. The rest of your Driver Option configs look correct though.
I use OTC to launch RDP for my Terminal Server users and on my Terminal Servers I use an application called "Splitview" to actually split the desktop into two independent screens (it also makes it so dialog boxes and stuff don't appear in the middle of the screens).
