#20643: Receive chain goes deaf and does not recover

 Configuration:
 * BB 14.07 r42549
 * OLSR

 Lay-of-the-land:
 *  Ubiquiti Rocket M5 XM with 19dBi 120 degree sector panel high mountain
 location
 *  3 ad-hoc connected stations:
      1) Ubiquiti 23dBi AirGrid M5 XM H-pol (1) @ ~14 miles
      2) Ubiquiti 23dBi AirGrid M5 XM H-pol (2) @ ~10 miles
      3) Ubiquiti Rocket M5 XM with 30dBi RocketDish @ ~8 miles
 * Channel 149
 * Reproduced on 5Mhz, 10Mhz, and 20Mhz channel width.
 * Unable to reproduce with Attitude Adjustment
 * High RF congested metropolitan area in SoCal Orange County.  Possible
 noise source triggering the cycle.
 * Goes to deaf state 1 to 3 times a day.
 * Co-located Rocket M3 and M2 with respective 120 Sector panels.

 Symptoms:

 On the mountain Rocket M5, the H-pol antenna chain RSSI drops to ~noise
 level and remains stuck.   AirGrid OLSR links drop with intermittent ad-
 hoc connections.  Intermittent cross over of H-pol transmitted to V-pol
 receive with AirGrids.   "OFDM LEVEL" remains at 8/9 or 0/1.

 Deaf State RSSI:
 RocketDish
         signal:         -69 [-91, -69] dBm
         signal avg:     -68 [-92, -68] dBm

 Airgrid (1)
         signal:         -85 [-97, -85] dBm
         signal avg:     -84 [-97, -84] dBm

 Airgird (2)
         signal:         -89 [-97, -90] dBm
         signal avg:     -88 [-97, -89] dBm

 Normal State RSSI:
 RocketDish
         signal:         -66 [-69, -69] dBm
         signal avg:     -65 [-69, -68] dBm
 AirGrid (1)
         signal:         -75 [-75, -85] dBm
         signal avg:     -74 [-74, -84] dBm
 AirGrid (2)
         signal:         -74 [-74, -90] dBm
         signal avg:     -74 [-74, -91] dBm

 Work Around: run an "iw wlan0 scan" and deaf state returns to normal
 state.

 This issue is affecting our http://www.aredn.org release timeline.

 Related Notes:
 1) we have removed all of our possible related code (extended part 97
 channels) to test and isolate this issue.
 2) Similar symptoms, but limited data points, have been observed on 2Ghz
 Bullet M2 and NanoStation M2 devices.  Links dropped out and "iw wlan0
 scan" recovered.
