To get the precision for packetloss and jitter for monitoring VoIP you pretty much need persistent polling of the path. So Nagios/Opsview would not be a suitable polling agent, but you would instead query something else that is doing the persistent polling.
For Cisco equipment you can enable SLA monitoring on the devices on key paths then monitor through SNMP counters. I saw some examples of this on a Cacti forum I believe, so I started working on developing some plugins for SLA monitoring but that work is still pretty raw right now (I had a tight deadline to show we could use Opsview to VOIP monitoring). But if you are dealing with Cisco stuff snmp polling of SLA monitoring seems to be the way to go. Here are some examples of info gathered by snmp polling of SLA paths..
