You don't indicate which version of Oracle you're
running, but the most common reason is that the
services have gotten corrupted. Usually recreating
the services will resolve this (see Note: 61621.1 on
metalink).
There were also some bugs in older versions where tabs
in the init.ora file would prevent oradim from being
able to startup the db.
