A few different scenarios

1) missing indexes .... will have slower performance than possible

2) invalid indexes ... will not be used, and again will have slower
performance.
(indexes can be invalid due to failing direct loads, moving tables....)

3) corrupt indexes ... unpredictable results. Often you will get
errors (like ORA-600 or corrupt block), but I have also seen where
the result set was wrong and no indication of a problem
(typically manifests itself in different number of rows returned
when do select statement and do the same select modified
to invalidate the use of indexes (old PK+0 or PK || '' trick).
