I ran into this problem before. oracle had crashed and we could not restart. So , If oracle is truely shut down on that box do an ipcs -m to show all active memory segments.
Find the ones owned by oracle and do an ipcrm -M key and remove the shared memory . I did then , then the database could restart just fine.
