I was getting that when I tried to select accros a DB Link when my Database and my Client Code were at different levels.
In our case, Database 8.0.5 and Client Code of 8.1.7 caused a failure. Since we upgrade our database to 8.1.7 all is well. What are the versions of your client code and database ??
