Do you install your form 5 in the same directory with oracle 9i ? If 'yes' your registry in your OS is already changes, that's why you can't start your 9i. Perhaps it is a bad idea if you reinstall your 9i (but check whether there is a facility to repair your oracle 9i on its cd) or you reconfigure your registry in OS. I wonder why you install form 5 in your server, as far as I know form is intalled on client not in server. Hope this helps.
