Dave,

When I've seen this, it's usually because I have added a new database on the
machine and the various SHM kennel settings are no longer appropriate for
all the instances.  Have you added a new instance since the last time you
restarted this instance?  How long has the machine been up, it could be a
contiguous allocation issue too.  I've sometimes noticed that if the kernel
values are close to the limit, then the order things get started after a
reboot can make it work sometimes and not others.

After my nightly cold backup and during the database startup I got this
error;

ksedmp: internal or fatal error
ORA-00600: internal error code, arguments: [3051], [82], [73], [], [], [],
[], []
ORA-04030: out of process memory when trying to allocate 8389132 bytes (pga
heap,THREAD RECOVERY)

The database mounts but does not startup.  I already filed a TAR with Oracle
but was wondering if any of you might have a clue on this one.  This is
suppose to happen on Monday, not Friday.
