Dave, When I've seen this, it's usually because I have added a new database on the machine and the various SHM kennel settings are no longer appropriate for all the instances. Have you added a new instance since the last time you restarted this instance? How long has the machine been up, it could be a contiguous allocation issue too. I've sometimes noticed that if the kernel values are close to the limit, then the order things get started after a reboot can make it work sometimes and not others.
