Not sure, maybe a pbm with the web.xml.

No idea at this time.

Just did a fresh download myself to try and repro this and was not
successful.  

For comparison's sake, here is what I did.
o downloaded 1.1.0 Studio from Source Forge, unzipped it, launched it.
  ( with jdk 1.4 on path. )
o selected File | Import..., OIS Application from WAR, specified
  - orbeon_prj1 as the project name
  - orbeon_prj1 as the context 
  - orbeon.war that was in the Studio zip that I downloaded.
o Selected Run | Run..., selected J2EE Container for OIS,
  clicked new, clicked run.
o In monitor perspective browsed hello world examples.

Did you do anything different?
