The title as shown on the image you sent has the following kanji on it:

千羽鶴 折形

Depending on how you read the characters you can have:

千       羽       鶴
chi     wa      tsuru
sen     u       kaku
sen     ba      zuru

and since the last two characters are "orikata" you indeed have the Senbazuru 
Orikata.
