During start_over, the failed images in /var/lib/systemimager/images were unable to be fully deleted. Turns out the proc file systems in each incomplete was somehow actually pointing at my box's live /proc. I had to reboot my box before I could delete the contents of /var/lib/systemimager/images/*/proc/. I found out it was my live proc when I tried to change permissions on a file and I saw the change happen in /proc as well. There was no indication that they were in any way linked (not symlinked, no extra mounts showing up in df), so I had no idea. Another weird thing, but at least a simple reboot solved it. --Joe
