So just a theory, but:
pfilter needs a restart after the nfs server gets restarted, because pfilter has to autodetect the random port that NFS chooses to open up the NFS service.  This might explain why restarting the server works.
This theory breaks down though, because I believe the default configuration is to have *everything* open within the cluster...
Either way, it's worth a try.
