This change is very much intentional.  osgViewer::Viewer isn't like
osgProducer::Viewer in that the later by default puts everything
including the kitchen sink into the viewer.  osgViewer::Viewer just
provides basic functionality out of the box, and you add the various
facilities you need.

The osgviewer example adds various extra event handlers so it behaves
more like the old osgviewer, it also adds facilities like being able
to change the threading model on the fly, but isn't a complete
implementation of the rest of the osgProducer::Viewer/old osgviewer
facilities. One of missing elements is the toggle between full screen
and windowed mode. This will come.

These extra features won't be back ported into osgViewer::Viewer, or
the other examples unless specifically required.  The focus of the
rest of the examples is the illustrate and test specific OSG features
and adding lots of viewer features just distracts from this.
