`qgis-ltr.bat --postinstall` called from the postinstall should have
created the `.env`. So I'd continue with
[https://github.com/qgis/QGIS/issues/26891#issuecomment-495889169 QGIS
#26891] and check if anything stands out in `%TEMP%\qgis_app.log`, when
`qgis-ltr.bat --postinstall` is rerun.
