This has to do with the world_map which is used below zoomlevel 10. This
world map is english basedand nothing more.
Above zoomlevel 10 the local maps are used which contain city names in all
languages they have been maintained in.

So for small town there is simply a name without language tag which will be
the local name.
Big cities have names in many languages (when added) and can therefore be
displayed in local language or your language.
For London for example below a very short abstract (top name is the
"simple" name which is normally only attached to small cities):
<tag k="name" v="London"/>
<tag k="name:ab" v="Лондан"/>
<tag k="name:af" v="Londen"/>
<tag k="name:am" v="ለንደን"/>
<tag k="name:an" v="Londres"/>
<tag k="name:ang" v="Lunden"/>
<tag k="name:ar" v="لندن"/>
<tag k="name:arc" v="ܠܘܢܕܘܢ"/>
<tag k="name:arz" v="لندن"/>
<tag k="name:ast" v="Londres"/>
<tag k="name:ba" v="Лондон"/>
<tag k="name:bat-smg" v="Londons"/>
<tag k="name:bcl" v="Londres"/>
<tag k="name:be" v="Лондан"/>
<tag k="name:be-tarask" v="Лёндан"/>
<tag k="name:bg" v="Лондон"/>
<tag k="name:bn" v="লন্ডন"/>
<tag k="name:bo" v="ལོན་ཊོན།"/>
<tag k="name:br" v="Londrez"/>
<tag k="name:ca" v="Londres"/>
<tag k="name:cdo" v="Lùng-dŭng"/>
<tag k="name:ckb" v="لەندەن"/>
<tag k="name:co" v="Londra"/>
<tag k="name:cs" v="Londýn"/>
<tag k="name:cu" v="Лондонъ"/>
<tag k="name:cv" v="Лондон"/>
<tag k="name:cy" v="Llundain"/>
<tag k="name:de" v="London"/>
<tag k="name:diq" v="Londra"/>
<tag k="name:el" v="Λονδίνο"/>
<tag k="name:eml" v="Lòndra"/>
<tag k="name:en" v="London"/>
