I can't explain the strange behaviour you talk about, but OsmAnd registers 
itself as a device administrator because otherwise it cannot turn the 
screen on when it is off. (This is useful when you're following a road for 
a long distance and don't need to have the screen on--OsmAnd switches it on 
when you approach a turn.)

Changing audio settings is most likely so OsmAnd can interrupt music to 
play its announcements over Bluetooth.

I doubt there is anything sinister going on. If there was, others would 
most likely have reported it by now.
