Hi,
Without using geoid and Digital Elevation model, it is no wonder that
the ortho-rectification is innacurate. Why ? You are basicelly
considering that the line of sight of a pixel intersects the earth at
the altitude 0m above the ellipsoid (note that this altitude can be
changed with the elev.default parameter), which is obviously pretty
wrong most of the time.
I recommend you to read the following sections in the cookbook :

https://www.orfeo-toolbox.org/CookBook/CookBooksu38.html#x59-810004.2.4
Which will explain how to perform the ortho-rectification step properly,
using DEM and geoid.
