Hi David,
regarding the second problem, you're right the OTB development packages
are required to compile and run examples.
You need to do : apt-get install libotb-dev.
Note that I've submitted a pull request to OSGEO-Live to integrate the
package in the next version:
https://github.com/OSGeo/OSGeoLive/pull/51/files
Regarding hte third problem, you need perhaps to set the cmake option
ITK_DIR in the HelloWolrdOTB project to point to /usr/cmake/itk-4.6

Solution: installed cmake using apt-get install

(includes for itk in /usr/include and libs of ITB in /usr/lib/
