Answering myself: this happens because I moved the OTB-5.4.0-Darwin64 folder to a different place. The doc is actually very clear: "You are allowed to move these .app files and refrain from moving or deleting OTB-5.4.0-Darwin64 after extraction. In case you need to have OTB installed in some other directory. Extract the .run file there."
