Hello Barbara,

I see patients with tbi in the rehab setting. Most of the time, i find that
activities that are most meaningful to them. I totally agree with Ron regarding
engaging in occupations... and also lynna  re: using a scale to gauge your
patient's level of cognitive/conscious level. We use the rancho los amigos here
too!

You will need to find out what is the level of function and what is the
occupation to engage the person in. furthermore ..  you would know the what
would the structure of the session (set up, amount of cues or breaking down the
task to decrease confusion) or activities you want your patient to do.

btw what do you mean tremors associated with TBI injuries? is the tremors due to
the neurological insult; clonus, intentional tremors, ataxia, dysmetria? or just
plain muscle weakness? i find the book functional approach to stroke rehab by
glen gillen and somebody... a good place to start my research.

Depending  on  the  patient's abilities, probably the best strengthening
you  can  do  for  this person is to engage them in occupations of daily
living.  For  example,  if  the  client is too weak to stand and pull up
their  pants, then work on standing. Or if the client is to weak to walk
to  the  toilet  then  build  their strength by walking. In other words,
specifically  work  on the occupations which are limited by the client's
decreased strength.

I would like to help you out, but your presentation of the client is very vague.
I would start with finding what level of recovery your client is at right now
(try the Glascow Coma Scale or Rancho Los Amigos or whatever scale your facility
is familiar with).  From there, it will be easier to perform an activity
analysis and come up with appropriate activities for your client.  Hope that
helps to get you started.
