Pat,
I would be glad to give you some suggestions, but let me ask this first. What is the dx and are there any precautions. Have you done an in depth interview with this person:ie COPM and learned of their everyday life tasks?I find that this is the answer to tx media.For example does this person work? do they type, use a push button phone, ...cooking, reaching for objects at grocery store, kitchen shelf,etc.

This is occupation-based practice at its best!But remember to ask this person to give you the daily life tasks that area a problem...you'll have to facilitate with open ended questions and have them break down a typical day.

There are many everyday tasks that require finger extension.  Are you using the task oriented approach to your assessment and treatment.

I am looking for new ideas for increasing finger extension (ring and
middle) in a patient that fell and landed on her hand.  I can only think of
about a half dozen, and it gets old doing the same things over and
