It depends on the purpose of the screening checklist.  Are you planning to do this in conjunction with the teachers? They're often the best source of information with regards to the children.  In a practice I previously worked with we considered screening children and designed our own screening which consisted of a basic interview form for teachers and parents and then used the Beery-Buktenica test of Visual Motor Integration (VMI) as a screening together with general observations of physical performance.  Have a look at normal development to guide you.  I'm sure there are screening tests out there that others have used, you may consider contacting other OT's in your area for advice or your school of OT for advice.  You may also want to check up on the ethics of the screening of these children, will you be following up on all of the children that are 'screened' as having needs for OT? Is this in Private practice or as a OT in the school?
