I would recommend the BiVABA by Mary Warren for assessment. She has done some workshops on this and also has good suggestions in the AOTA Self-Study Series: Cognitive Rehabilitation , Lesson 7 as well as a publication in the American Journal of Occupational Therapy, 44, 391-399 and 47, 42- 66 . The BiVABA is available through www.visabilities.com . If you want other references, I have quite a few.
