Hey Shellie, It might be effective to explore pain managemnt strategies. Determining up and down time relative to the ability to garden may help (up time is how long you can do an activity without a significant rise in pain or fatigue and down time is the time it takes to recover from up time. Explore modification of the gardening to provide like results but removing the offending components (i.e. since it is spinal stensois - bending). Has the patient tried TENS? It may also be helpful to instruct on posture, body mechanics, energy conservation and work simplification.
