I have renewed my NBCOT certification and I intend to keep renewing it.  My 
current employer requested it, and in order to become a provider on our (my 
employer's) insurance I think I had to have it.  I am not sure if all insurance 
companies require NBCOT certification.
Also, when a classmate of mine moved to Colorado after graduation and she had 
not taken her boards, she had a hard time getting a job because at that time (I 
don't know if this is still true), OT's were not licensed by the state and most 
employers would only hire an OTR.  
I don't know what the requirements are for the state in which you are licensed. 
 Personally I figured that it was easier to maintain than to let it lapse and 
have to renew for whatever reason.  Hope that I have been helpful.
