Ron, Sorry to hear of the struggle and I think a lot of us have been there before...getting the "OT slap in the face." We are the Rodney Dangerfields of the Rehab worll...we get "no respect...no respect at all". You have my admiration for your conviction to your ethics and principles which is all you have in the end. The goal is 30 visits/week and I've only been as high as 24. I guess I'm to blame because I give patients ONLY what they need. If a patient needs 5 day/week, that what they get. If they don't need any OT, then that's what they get. I guess I could just see all patients 3x/week and that would take care of the productivity, but I don't feel that's appropriate. I told my supervisor that I can not see patients if I don't get the referrals. I told my supervisor that I've always thought OT should be the premier discipline but I don't think she agreed. I also had to counter the OT = upper extremity "thing" at least three times. It seems that no matter how many times I explained that my OT is not about UE, she just didn't get it. She did ask me to do an inservice for the nurses, but I'm not too optimistic... I must say that my poor little OT feelings are deeply hurt. The agency just hired 2 new PTA's. I've noticed a real shift over the past 15-20 years in the UK as more and more OTs have got into management positions and damn well made sure that the OTs are now influencing and dictating corporate strategy and policy decisions. I know that this doesn't help Ron in his immediate position and obviously I can't speak for the American Healthcare system but when I first started as a humble OT assistant (way too long ago), the vast majority of managers were either social work trained or had a nursing background. Thankfully this has changed (our new Adult Social Care Director is an OT) as there was a professional consensus to actively get in there and change things. Personally I was really worried that I'd be selling my professional soul if I got into management but I'm applying OT principals to my work and am making sure that the OT voice is being heard.
