Yes, it does work. We're running it on OS X server.

Does  OTRS need to be re-complied for OSX 10.4 ?

It's all written in Perl, so you don't need to recompile. It's possible that the install might not be quite as smooth as it is on e.g. a Linux system; in particular, Server Admin and I have had a number of fights over Apache settings…
