Hi, I got something similar, and if I remember correctly, it seems that the password or user name for the otrs user to access MySql db is incorrect. Try to logon directly to MySQL with the otrs user and see if you can. If you can do so, then check the config.pm and check that the password is correct in the section for Database settings.
