The basic path is set in the Apache configuration file and should be something like "/opt/otrs/var/httpd/htdocs/" with the actual image paths underneath that being in the Standard theme. See also the Admin Manual section 3.2.3. It's also worth noting that the image paths might be correct but not the images themselves; I had this issue come up after a manual upgrade wherein a sed substitution corrupted all of the image files; recopying them from a fresh install resolved this.
