After an upgrade you would need to re-do any customizations you make
to OTRS files - including to templates.
That's why the manual recommends you would create your own theme
instead of customizing the default theme. However, there's also a
drawback to that. If the default theme would change in the new
version, you would miss those changes after the upgrade. This might
affect the functioning of OTRS.

So in my opinion it is best to keep track of the customizations you
make to OTRS files, back them up and re-apply after an upgrade.
You can use all kinds of fancy diff viewers and other tools to help
you with this.
