This is a known problem of 3.0.4 and it can happens if you have an
attachment dragged to an email instead of using the attachment button.

I used to have the exact same issue. The point is that attachments
which have a link to them in the body of the mail are discarded in the
attachmentlist in OTRS. So attached images which are shown in the body
do not appear in the attachment-list.

This is fixed in 3.1.x and maybe even earlier. The temporary fix I did
back then was that I commented out the code to hide these "used"
attachments.

If you check the attachment-table you will probably see that the
attachments are stored there.
