Besides the $410, You need to bring your health care information Emergency contact information email address phone number where we can contact you during the holidays A pen (makes the filling out of sheets a little faster) For those of you who are visiting Canada, please make sure that you are able to actually travel in the US before you sign up. I think you need some sort of VISA.
