I don't have any native modules.

It's so hopeless that I suspect I am attempting to do something that is not 
possible. Perhaps assuming that a managed module could be applied at the site 
(or whole IIS) level is erroneous. All of the instructions I have seen, 
including yours, require a "reference" to the module. When I try to add the 
module to a site it says "The specified type cannot be found among the 
referenced assemblies for this application". At the site level I have no app 
and I have no config file.

I must therefore be trying to do the wrong thing with the wrong tool.

When I used to add my C++ ISAPI handler in previous years I added it to IIS 
itself, not a site. Is there a managed equivalent? Hmmm
