The report you are seeking is called "Having a Baby in Europe" published by 
the WHO in the mid 80's.  It looked at pregnancy care in the 32 countries 
in Europe, and was the preliminary report that led to the the 3 WHO 
Consensus Conferences on Appropriate care for pregnancy, birth abnd post 
partum. These three meetings (the story of how they were convened, the 
research they used and the recommendations that were adopted by the WHO ) 
are all described in "Pursuing the Birth Machine" by Marden Wagner (he 
chaired the meetings).
