I too suffered terribly from nipple thrush with my first child.  It presented itself within 1 week of delivery and I suffered until approximately 16-17 weeks.  Sometimes I wonder how I managed to continue feeding - the pain was so bad.
 
The LC I saw (this was over 5 years ago) offered a cure that no other could.  She encouraged me to buy acidopholus too or Herbalife Florafibre 3 times daily.  She additionally suggested 1000mg vitamin C taken 3 times daily and then decrease to 1 upon healing, mega B supplement 2x 3 daily, 1000mg evening primrose oil and 1000mg calcium supplement.  She told me to swab the nipple and areola with a baking soda and water solution and then apply daktarin and a nipple moisturising cream like Marcalan or Pawpaw after breastfeeding.  She also encouraged me to go and see my GP and ask for a script of oral nystatin (Mycostatin) 2 million units 3 times daily for 4 weeks and use topical nystatin ointment or Canesten cream twice daily. But the main thing that contributed to killing it was cutting out sugar.  All sugar, honey, dried fruit, cakes, alcohol, fruit juices etc.  I also substituted whole grain flour for white flour etc - eat high protein.
Additionally I either boiled all my underwear, cotton nursing pads and bras or soaked in Napisan.  I also cleaned the shower and bath with White King after I used them.
I know it all sounds a little obsessive, but it worked for me.
