I suspect it is more of a case of that is the status quo as well as they pit a test dose in to be sure it is correctly placed etc.
It may be difficutl to do a test of effectiveness with say saline??
Perhaps yopur client needs to consider asking more about what are the possible (evidence based not fear based) consequences for herself and babies  if
1) they give her an epidural
2) they donot
 
at what stage of the labour and for what indications?
 
Also do some surfing, searching  for a full range of options about care of a twin pregnant-labouring (I presume healthy ?) woman and take this information when you go to discuss things with your doctor if you want ??

Hi all
 
I have a client, 42 y.o first time mum who is expecting twins. Her ob/gyn has told her that he would like her to have and epidural anyway to prepare her in case they have any trouble with the second twin being born. When she asked the midwife at her antenatal class if they can just insert the needle without giving her the actual medication she was told that it was either give it or not prepare her for the epidural at all, no half way.
 
In your opinions is this correct or can they do all the prep and then administer the drug if complications occur?
